When you wish to register with the Practice you will be asked to complete a GP Registration Form, a New Patient Questionnaire and a Texting Consent Form. We will ask you for proof of identity.
You must live in the G71 5**, G71 6**, parts of G71 7** and G71 8** postcode area (Please note our boundary does not extend beyond the A724 Three Tins roundabout) to be able to register permanently with Bothwell Medical Centre. Please read our registration policy below.
Although we process applications to register as quickly as possible it will take up to two weeks for your registration to be completed. Your medical records may take several weeks to arrive at the Practice, especially if your last practice still held paper records.

You can send the forms by Royal Mail, post through the letterbox at the front door of the practice or email them to [email protected] . Remember to send in copies of your ID too or we won’t be able to process the application.
