From 03rd August 2015 Bothwell Medical Centre are  


changing the way patients can request a repeat


prescription. Our Patients will no longer be able to order their


repeat prescription using a pharmacy managed repeat system.


 


Patients who have a weekly dispensed dosset box or bubble pack managed by the pharmacy will remain unaffected by this change. Your preferred pharmacy will also still be able to collect your prescription from the surgery on your behalf.


 


 


You can order your repeat prescription using any of the


following options


 


1: By using our repeat prescription voicemail system


01698 854583. This system is available 24 hours a day 7 day a week


 


2: By using our online prescription request service via our practice website www.bothwellmedicalcentre.co.uk


 


3: In person or by post.

New Patient Registrationregister_now

 

If you would like to join the practice, one of the receptionists will give you a registration form. Persons 16 and over shall also receive a general medical history form to complete. You will then be registered with the practice and may choose to consult with any of the GP partners.

All new patients who register with the practice must have an initial health check before registration is confirmed. This is a free health check for you and an opportunity for us to obtain important medical history prior to your medical notes arriving. The receptionist will book your medical with the practice nurse and you will be asked to bring along a specimen of urine.

Medical treatment is available from the date of registration. Please contact reception for further information.

You may print off a registration form and a new patient questionnaire , fill it out and bring it in with you on your first visit to the practice. To register with the practice you must make an appointment with the Practice Sister. We also require two recent proof of residence documents (e.g. utility bill, bank statement, community tax).
